OSHA Log of Work-Related Injuries and Illnesses, Form 300

OSHA Form 300: Log of Work-Related Injuries and Illnesses lists all work-related injuries for a calendar year and categorizes the severity of each case. Companies in the US must yearly complete this report and note any workplace injuries that meet OSHA criteria. The Archibus Incidents Log report presents all the data that you need for this OSHA report. Therefore, you can generate the OSHA report directly from your Archibus data. The Archibus system will generate a PDF that emulates OSHA Form 300 and will complete the various fields of OSHA Accident Report (Form 300) with the data from your workplace incidents.

  1. Run the Management Reports / Incidents Log Report.
  2. Generally, you do not want to enter a restriction in the Filter panel at the top of the view because the OSHA report will not consider this restriction. Therefore, leave the Filter panel empty and click Show.
  3. When the Incidents Log report displays, click the OSHA Log Report button in the upper right corner.
  4. The system presents a dialog so that you can enter the reporting year and other optional criteria (Country, Site, Building). For example, you may want to submit one OSHA Form 300 for each building, each site, or one form for all sites within the country. Since the OSHA Form 300 is based on calendar year, be sure to choose the appropriate year. Click OK on the dialog.
  5. The system generates a PDF of the OSHA Form 300 and completes the relative fields with data from the Archibus system. Click on the PDF link to load the generated PDF. For more information on long-running jobs such as this, see Running Jobs
  6. Review the information and print the PDF. Note that you will need to manually complete the Establishment Name, City, and State options in the upper right corner of the report.
  7. For each entry on OSHA Form 300, you must file OSHA Form 301. For information, see OSHA Accident Report, Form 301.
