Questionnaire Samples

The sample HQ project contains several sample questionnaires that you can use as a model when developing your own questionnaires. These samples are derived from real-life surveys and include the types of questions you will encounter in conducting your own surveys. Before developing your own questionnaires, you will find it beneficial to take some time to explore these samples and see how the questionnaire tool can be used to address a variety of survey needs. Review the questionnaires in Preview mode to gain a sense of the many different ways you can present questions on your questionnaires.

When you see a format that you want to emulate, you can look it up in the Define Compliance Questionnaires view and see how it is structured. For example, suppose you peruse the Permit Renewal questionnaire, and want to examine how the multiple choice questions were established. You can search for the multiple choice questions for this questionnaire and then view the details in the Question Display Options panel.

The "Fire Doors - self closing & latching" questionnaire (ID=9) that contains only a few questions and is an example of questions that you can use in multiple surveys for multiple compliance requirements. These questions reflect the NFPA 80 regulations and might be used as part of hospital environmental tour survey, a fire inspection survey, or general safety survey. The questionnaire is set to Inactive as the questionnaire is designed to hold the questions that you will assign to other questionnaires.
