Using Printed Forms for Field Assessments

If your site does is not able to provide field assessors with laptops or mobile devices for recording their observations, you can print a report listing the items to assess and have the field assessor record their observations on the hard-copy paper.

After an assessment manager assigns items to an assessor, follow this basic process to work with hard-copy printed forms:

  1. Print out the assessment items for an assessor to take to the field. Typically, there are two possibilities, as listed below. Note that both the assessor and manager can use the Filter option of the Manage Assessment Items tasks to limit the number of items included in the report. For example, a field assessor may wish to print only items for a certain floor; a manager may wish to print only records whose status is N/A.
  1. So that assessors can jot down in their notes the typical resolutions to problems they encounter, you may wish to print out a report of defined problem resolution codes, which is available from the Reports task.
  2. Assessors in the field bring the report with them to the field and visit the items listed on the report. On the hard-copy report, they record their observations of each item listed on the report.
  3. When field assessors finish the assessment, they do one of the following:
